First Visit

Please arrive 15 minutes early for your appointment in order to complete all necessary paperwork and allow us time to process your information. Please bring any dental insurance information as well as the following forms to streamline your visit.

Your initial exam should take approximately 30-60 minutes, during which your doctor will provide an evaluation that outlines any existing conditions and possible treatment. You will also receive a dental cleaning and oral hygiene instructions with suggestions to help you care for your teeth.

At all times, we encourage you to ask questions, tell us how we’re doing and let us know how we can be more helpful to you. Our team is here to help make your visits pleasant, informative and positive.

Discounts and free second opinions

We offer a cash discount, senior discount, and DLM Club Member discount. In addition, financing is available for 12 months at 0% interest.

We care about you and believe that a well-informed patient is a happy patient. That is why we always offer complimentary second opinions.

Cancellations

Your appointment time is reserved specifically for you. We request 24-hours notice if you need to cancel your appointment.

Financial Information

We pride ourselves in our honest approach to your treatment plan; every patient is fully educated on our treatment options including cost breakdowns. Prior to treatment, we will discuss your treatment plan, including a breakdown of all applicable fees, and we will inform you of all costs.

Costs are payable at the time of treatment. Please ask a member of our staff, and we will review our payment options to accommodate your financial needs. If special arrangements are needed, please talk to our office manager prior to receiving service.

Additionally, we will submit all insurance claims on your behalf and will try to help you receive full insurance benefits. However, you are personally responsible for your account, and we encourage you to contact us if your account has not been paid within 30 days.

Insurance

Our office is committed to helping you maximize your insurance benefits. Since insurance policies vary, we can only estimate your coverage in good faith, but cannot guarantee coverage due to the complexities of insurance contracts.

Your estimated patient portion may be paid at the time of service. As a service to our patients, we will bill insurance companies for services and allow them to render payment. Your personal balance is due within 30 days after the insurance company has processed the claim.
